Support services are designed to help seniors remain independent as
long as possible. Our various service programs provide assistance
with activities that are essential for seniors wishing to remain
safely in their homes as well as being able to get to needed
services. We are also here to support family, friends and care
givers who are vital to the independence of many seniors.
Our
outreach case workers are often the first contact in arranging
needed services. Outreach provides an important link between
seniors and the services of the Agency on Aging, as well as other
assistance programs and resources. Our licensed social workers meet
with senior adults, age 60 or better, especially the homebound or
isolated, to evaluate their needs and connect them with the
appropriate agency services, community service organizations and
resources. Outreach services include:
Needs assessments help us
identify what services, resources or assistance programs are
most needed by the seniors or caregiver.
Assistance in accessing services,
resources or assistance to seniors and their caregivers. This
may include help with paperwork, scheduling, telephone serving
as a liaison and otherwise advocating for senior welfare.
Transportation
assistance/coordination for medical or other critical needs.
Caregivers training are offered
periodically to assist caregivers in connecting with a variety
of resources to support them in caring for aging family or friends. For more information,
